    Identity and Context

    Provider

    The billing provider for the row. Click the chevron to expand/collapse row details (e.g., code description).

    Date of Service

    Shown as M/D/YYYY.

    Code and Code Description

    The billed code. If modifiers were entered, they are appended with slashes (e.g., 90371/26). The description of the code (if matched to a fee schedule) appears in the Code Description field.

    Under some sources (e.g., Part B Drug), an additional detail in the Code Description may appear beneath the row (e.g., “per 1 mL”).

    Source

    Shows which pricing table was matched (if any). If no match found, or if the date of service predates the fee schedule implementation, this column shows No Fee Sched.

    Units

    Quantity for pricing. Its meaning depends on the source (e.g., miles for ambulance, minutes for anesthesia, quantity for most services, drug-specific units for Part B).

    Charges and Reductions

    Gross Charge

    The provider’s billed amount before reductions.

    Adjustment

    Reductions you entered (contractual write-offs, courtesy discounts, credits).

    Net charge

    Gross Charge − Adjustment.

    Schedule Amounts and Payments

    Medicare

    The base fee pulled from the applicable schedule before any multipliers or statutory logic.

    This field may not be applicable if a fee schedule does not apply (or is not used for the date/jurisdiction).

    Fee Schedule

    The allowed amount after applying the correct schedule and multipliers.

    In the same cell, you will also see an explanation as to how the figure was calculated (for example, “Medicare × 190%”).

    Insurer Paid

    Amount already paid by the insurer (entered on Edit Project).

    Balance

    Fee Schedule − Insurer Paid.

    How the Math is Determined (at a glance)

  • The system first queries multiple fee schedules.
  • If a fee schedule is found:
  • The Medicare column shows the base fee from that schedule.
  • The Fee Schedule column then shows the allowed amount (and nonparticipating adjustments, when applicable).
  • If no fee schedule applies:
  • The system applies the jurisdiction’s fallback rules to either the CDM Price (Michigan specific) or the Net Charge, then displays the result in Fee Schedule column and sets the Source to No Fee Sched.

    To see the full report:

  • Open your file in Excel.
  • Look for the yellow notification bar at the top of the spreadsheet.
  • Click Enable Editing.
  • Once enabled, all rows and totals will appear as expected.

    Project Information

    At the top of the page, you will see your project’s key details, including:

  • Project Name – The name you assigned to the project.
  • File Number – Your project’s identifying number.
  • Last Modified – The most recent date changes were made.
  • Project ID – The shortened unique ID for your project.
  • This information is displayed for quick reference when reviewing reports.

    Report Views

    The report has two different tabbed views you can switch between. Switch between the tabs to adjust the level of detail you wish to see.

    All Totals
  • Displays overall totals across all providers included in the project.
  • Useful for high-level review of total charges, insurer payments, and balances.
  • Provider Totals
  • Breaks down totals grouped by provider.
  • Useful for comparing and analyzing performance or billing patterns across different providers, or for settling individual providers.
  • Report Columns

    The report displays the results of your project entries. To make edits, return to the Edit Project page. Below is an explanation of each column shown.

    Export Options

    You can export your report for external use in two formats:

  • Excel – For spreadsheet analysis and recordkeeping.
  • PDF – For printing, sharing, or inclusion in case files and exhibits.
  • Export buttons are available at both the top and bottom of the page for convenience.

    Editing Projects

    If you need to update your billing data or adjust fields, you can return to the project editor by clicking Edit Project. Once updated, the Fee Schedule Pricing Report will reflect the new data.
